How Referrals Work

After receiving the referral, our team reviews:

• Patient location

• Service availability

• Care setting

• Referral needs

• Required provider orders or documentation

• Appropriate next steps for scheduling

We will communicate with the referring organization if additional information is needed before services can begin.

Service availability may vary by location, clinician coverage, patient needs and clinical appropriateness.

Before a visit, the team may request appropriate clinical information to support safe, coordinated care.

Depending on the situation, this may include:

• Patient demographics and contact information

• Referring provider information

• Relevant medical history

• Current wound-care orders

• Current medication information

• Recent wound documentation

• Relevant laboratory or diagnostic information

• Insurance or authorization information, when applicable

• Facility or home-health contact information

Please use only approved, secure channels for protected health information.

Once the referral is accepted and scheduled, a qualified clinician visits the patient in the approved care setting.

During the initial evaluation, the clinician may assess:

• Wound type and location

• Wound size, tissue condition, drainage, and surrounding skin

• Pain and comfort needs

• Current treatment approach

• Healing barriers and relevant risk factors

• Patient mobility, care environment and support needs

• Need for ongoing follow-up or additional coordination

The evaluation helps determine the most appropriate next step and care plan.

Following the evaluation, Partnered Wound Care coordinates with the appropriate provider and care team to establish an individualized treatment plan.

The plan may include:

• Recommended wound-care approach

• Dressing selection and change frequency

• Follow-up visit schedule

• Monitoring goals

• Facility or home-health care instructions

• Communication and documentation expectations

• Recommendations for additional evaluation or escalation when needed

Care is provided in coordination with the appropriate providers, orders and applicable clinical requirements.

Wound-care needs can change quickly. Our team provides follow-up visits as clinically appropriate and communicates relevant updates with the referring organization and appropriate care-team members.
