Once the referral is accepted and scheduled, a qualified clinician visits the patient in the approved care setting.
During the initial evaluation, the clinician may assess:

Patient mobility, care environment and support needs

Following the evaluation, Partnered Wound Care coordinates with the appropriate provider and care team to establish an individualized treatment plan.
